Your First Visit
As a patient of AOMS Consulting Suites, we require essential personal information and details about your general health and well-being. This will ensure that we are able to provide you with the most appropriate treatment while you are under our care. For this reason, it is important that your practitioner has all the information necessary to plan and discuss your treatment at your first appointment.
The following information is important for your first visit:
Referral from your dentist/doctor
If you have been provided with a referral, please bring it to your appointment. Sometimes this has been provided directly to AOMS by the referring doctor or dentist. If you are unsure, please contact us, and we will let you know if it has been received.
X-rays
Your practitioner requires at least one X-ray called an OPG. This is an X-ray that shows your teeth and jaws. Sometimes, this will already have been provided to AOMS by the referring doctor or dentist. If we don’t have one, we can arrange for you to have one taken before your appointment.
List of medications
If you have an extensive medical history, it may be a good idea to obtain a health summary printout from your medical practitioner to bring to your appointment.
Medicare and private health insurance cards
The fee associated with your consultation is payable on the day in full and able to be submitted electronically via Medicare or your Private Health Fund (depending on the practitioner you are seeing) with the rebate returned to your nominated bank account.
