Important Information for Referring Practitioners
To ensure that the practitioners at AOMS Consulting Suites can provide you and your patients with the best possible treatment plan, we kindly request the following information for all new patient consultations;
Letter of referral including your current treatment plan for the patient and details of your request for treatment. Please contact our office if you would prefer to use our printed referral pads, these can be sent to your office today.
If referring to an OMS Surgeon or dental practitioner, a current OPG (within 12 months) or periapical radiograph of the area of concern. (Please retain ‘teeth’ ions for dot points)
